The only "legal" but extremely ugly and usually useless way to achieve what you want is to show Call Log to user. And I mean literally show (becomes visual, gets focus). In case you want to do this, here's how:

Because of overall performance. You need to somehow notify those apps that Call Log has changed (seen means changed, as well) and that it should update it. It would be nice if all such apps on all devices would receive a broadcast in order to refresh, but as far as I know, it's not the case.
I hope someone will find a better way to force refresh on stock phone apps.

Initiate a call
It is best to make calls from your app whenever you can. This is the best way to find where the call came from.

The truth about a call comes from a PhoneCall object.

PhoneCall.events can be read as a stream, and when the call is over, the plugin will gracefully close the stream. The plugin keeps an eye on all calls in progress and will eventually force any call to time out.

Watching all events
You can watch all the events instead of just focusing on one call. We recommend using “FlutterPhoneState.phoneCallEventStream” because it includes our own tracking logic, call timeouts, failures, etc.

I am developing an application for my friend who is in sales, this application will make phone calls one after another, as soon as one phone call gets disconnected, it will automatically make call to another number from the list. This list can be read from and xml data source or json or mongodb or even from excel sheet.
This could be an ios app that reads data from an end point and stores them and can initiate the call at any point and it wont stop until all the calls are made.
Next call will be made only after the first call has been finished.
I am thinking about using node based web app using google voice to trigger the chain.
I've no experience with ios / android apis but Im willing to work on that if it's a viable thing on that platform.
Note: what we're trying to avoid is whole process of
looking up the phone number.
touch hangup and then click for another phone number.
It should self trigger the next call as soon as current call gets disconnected.
Also we're trying to avoid any paid services like twillo.

I've been working on an android app concept in which the app has to auto-dial some special USSD codes in order to initiate certain telco services of interest to the user when the user initiates the service via a shortcut in the app.
The trouble I'm finding is that when the app tries to auto-dial such short codes or USSD numbers, the phone's OS (or is it the Call Intent), doesn't auto-dial, but instead presents the user with the code/number in the dial-pad and so the user has to manually initiate the call - which sort of defeats my intention of allowing users to initiate the services with just one click - the shortcut.
Currently, this is how I'm initiating these calls:

Actually, looking at another app in which I was autodialling user-specified numbers, the problem with the above code trying to auto-dial ussd codes was that instead of using intent.ACTION_CALL, I was using intent.ACTION_DIAL - which definitely just prompts the user with the number to call, without directly calling it. When I fixed that, the app now works as expected. See answer below...

Actually, despite what some people were claiming about Android preventing such a feature. When I looked at the code in one of my older apps which auto-dials user-specified numbers, I found the solution to be:

This works as expected - USSD codes get auto-dialled when above code runs. The only important thing to note when using this approach, being that you have to add the following permissions to your manifest:
<uses-permission android:name="android.permission.CALL_PHONE" />
So, as indicated in the update to my question, the problem with my original approach was using intent.ACTION_DIAL instead of intent.ACTION_CALL.

The PhoneStateListener's callbacks onCellLocationChanged and onSignalStrengthsChanged were the goto methods for when I wanted to handle cell and signal data changes in GSM and CDMA. With API 17+, I can see that there's a new callback (onCellInfoChanged) for handling both cell and signal changes.
Looking at the documentation, it's not clear what I can expect from the introduction of this new callback.
Will LTE changes always and only trigger onCellInfoChanged?
Will GSM/CDMA changed remain on the older callbacks?
Does one overlap with the other? (i.e. Both old and new get triggered for LTE or GSM/CDMA.)
It may very well be that different OEMs will have different implementations (sigh!), but I'm hoping there are guidelines that everyone's supposed to follow.
Can anyone shed some light on this?

I didn't test if but it looks from the code that both will be called.
I downloaded code source of Android 4.3(API 18) using the SDK Manager.
The following observations made me think that both would be called.
The class that triggers these events is: com.android.server.TelephonyRegistry
It notifies the listener though:

This same function calls for both type of notifications(Location and CellInfo) in a non exclusive way.

This one will call onCellLocationChanged even on new LTE phone since there is nothing from the above code that would prevent this. This needs double checking that there is no upper layer that filters the events themselves

There are other things from the code that look like CDMA will be calling the newer API. For example com.android.internal.telephony.cdma.CdmaLteServiceStateTracker seems to be dealing with CDMA and LTE. Again it would require a more careful look but that should give you a good place to start.
You can also try to simulate that with the emulator.
Using this code I managed to mark all missed calls as read:

You don't. That Notification is put there by another app, and you have no means of controlling whether that Notification is displayed, short of building a ROM mod that changes the behavior of that other app.
